The fitness number worth caring about

You can track steps, sleep and heart rate and still miss the one measure researchers care about most. VO2 max is having a moment because the longevity data behind it is hard to argue with.

It sounds technical. The idea is simple: how much oxygen your body can actually use when you push hard.

Why it predicts so much

Oxygen use at peak effort reflects how well your heart, lungs and muscles work together. That whole-system snapshot turns out to be one of the best single predictors of how long you live.

Research note

In a Cleveland Clinic analysis of 122,007 patients, published in 2018, low cardiorespiratory fitness carried 2 to 5 times the risk of early death, with no upper limit where more fitness stopped helping. Across studies, each one-MET rise in fitness, roughly a 3.5 ml/kg/min gain in VO2 max, links to about a 13 to 15% lower risk of death.

What it means for women specifically

Women tend to have lower absolute VO2 max than men, partly due to body composition and blood volume. That does not make the metric less useful for you. It makes the trend within your own numbers the thing to watch.

In fitness research that included women, each step up in cardiorespiratory fitness tracked with meaningfully lower mortality, including cancer deaths. The takeaway is the direction of travel, not how you compare to anyone else.

The decline you can fight

After about 30, an untrained VO2 max drifts down roughly 10% a decade, and the slide can steepen around menopause. That sounds grim until you see the other half of the data.

Training reverses a large part of it at any age. This is the rare area where the science is genuinely encouraging: your effort moves the number.

Practical tip

The most effective recipe is mostly easy aerobic work plus a little hard work. Build a base with zone 2 cardio a few times a week, then add one short session of harder intervals, a minute near-flat-out and a few minutes easy, repeated. Strength work supports it, as covered in our strength guide.

How to know yours

A lab test is most accurate, but most fitness watches now estimate VO2 max well enough to track your own trend over months. The exact figure matters less than whether it is rising or falling.

You do not need an elite score. Moving out of the lowest fitness band is where the biggest survival gains sit, and that is reachable for almost anyone who trains consistently.

When to see a doctor

If you are over 40, have been inactive, or have heart, lung or blood-pressure concerns, get cleared before starting hard interval training. Chest pain, unusual breathlessness or dizziness on exertion should always be checked promptly.
